VITA CENTRE

Vita Centre supports pregnant and parenting youth (ages 15 to 30) in the Region of Peel.

COMMUNITY COUNSELLOR POSITION:

One year maternity leave for 37.5 hours/5 days per week starting November/December, with possible extension.

 

JOB DESCRIPTION:           

The Community Counsellor reports to the Executive Director. The Community Counsellor will support the mission and values of Vita Centre to the pregnant and parenting youth we serve. Vita Centre serves the whole community within a Catholic values framework, therefore all candidates should have an understanding and commitment to the pastoral, moral and social teachings of the Catholic Church.

The primary role of the Community Counsellor is to assess client needs, assist clients with housing, financial needs, etc. The Community Counsellor empowers clients to set goals, refer clients to other services in the community and advocates on their behalf when necessary.

 

PROGRAM RESPONSIBILITES:

Caseload Management

·       Complete intake process, provide information, referral services and initial support to clients referred

·       Assess client needs and provide ongoing support and case management through Vita Centre programs & services

·       Guide and support clients in setting individual goals and objectives. Meet with clients regularly to evaluate goals & objectives and to set new ones

·       Connect clients with community agencies and volunteers that will support them in achieving goals (i.e. education, health, housing, employment)

·       Advocate for the clients when necessary

·       Refer clients to other services in the community when necessary

·       Assist in the facilitation of life skills, educational workshops or groups

 

Program Development

·       Plan community recreational and cultural activities for clients (past & present)

·       Plan new strategies to reach our target audience

·       Work closely with the Vita Centre team to develop community services based on the needs of clients

·       Educate the schools, community agencies and the public on the services Vita Centre provides

 

Agency Responsibilities

·       Compile monthly statistics on clients and submit a written monthly report

·       Evaluate client satisfaction with Vita Centre services on an annual basis

·       Support student placements with learning opportunities

·       Attend staff meetings and regular supervision with Executive Director

·       Involvement with relevant networking groups in the community, attend meetings and other functions on behalf of Vita Centre

·       Support the activities of the volunteers and Board Committees as required

·       Manage own clerical responsibilities

·       Other duties as assigned

 

Qualifications

·       Degree or diploma in Social Work, Child & Youth or related field

·       At least one year’s experience in human/social services

·       Client centred approach and ability to respect confidentiality

·       Sensitivity to issues around teen pregnancy

·       Knowledge of community resources

·       Excellent listening, communication and presentation skills

·       Well organized, able to work independently

·       Adaptable, flexible and resourceful

·       Proficiency in Microsoft Office, Excel, Outlook

·       Access to vehicle for travel to various locations across Peel Region

·       Completion of a satisfactory medical report

·       Criminal Record Check (within the past six months)
